This agreement is entered into between Shogen-Ryu Karate-Do International Limited (S.K.I.L. Dojo) and the
undersigned participant or their legal guardian. This binding contract ensures clarity and mutual
understanding of the rights, responsibilities, and obligations of all parties.
By signing below, I, , confirm the following:
1. I have been given the opportunity to participate in an Introduction Program or a prior yearly agreement
with S.K.I.L. Dojo, during which I or my child(ren) gained familiarity with the dojo’s instructional
methods, expectations, and operations.
2. I fully understand and voluntarily accept the obligations and expectations set forth by S.K.I.L. Dojo
regarding membership, attendance, conduct, payment terms, pictures/video use towards promotions
and other applicable policies.
1. MEMBERSHIP AGREEMENT AND DURATION
1.1 Membership is contracted for a 12-month period from the date of signing.
1.2 This agreement cannot be terminated or canceled except as expressly provided herein.
1.3 Cancellation Policy:
Cancellation requests must be submitted in writing or via verifiable verbal communication to the Chief
Executive Officer (CEO). Approval of such requests is at the sole discretion of the CEO. If denied,
membership will remain active, and all scheduled payments must continue as outlined or through a
lump-sum payment to satisfy the remaining balance.
1.4 Any modification to this agreement must be documented in writing and signed by both parties.
2. PAYMENT TERMS AND OBLIGATIONS
2.1 PAYMENT OPTIONS:
Payments may be made on a monthly or annual basis. Monthly payments are due on either the 1st or
15th of each month, as selected at the time of signing. Annual payments are due in full upon signing
the agreement.
2.2 ALTERNATE PAYMENT ARRANGEMENTS:
Must be approved in writing by management prior to the payment due date.
2.3 NON-REFUNDABLE FEES:
All payments are final, and no refunds will be issued for unused membership time or missed classes.
2.4 DELINQUENT ACCOUNTS:
A late payment fee of $35.00 will be charged bi-weekly for overdue balances. Members with overdue
accounts will be denied entry until all dues are paid in full.
3. ATTENDANCE AND MISSED CLASSES
3.1 Membership covers a full year of attendance.
3.2 Missed classes will not be reimbursed or carried forward unless otherwise approved by
management in exceptional circumstances.

4. FAMILY MEMBERSHIPS
4.1 Pricing for family memberships is determined by:
The number of memberships purchased for additional family members.
The chosen membership package.
The selected payment schedule (monthly or annual).
4.2 Management reserves the right to adjust family membership pricing based on dojo policy updates.
5. INSURANCE REQUIREMENTS
5.1 All participants are required to register with the Ontario Jiu-Jitsu Association (OJA) as a condition of
attending in-person classes.
5.2 OJA registration fees are separate from dojo membership fees and must be paid directly by the
participant.
5.3 This insurance policy applies to all programs, including Karate and Jiu-Jitsu.
6. WAIVER AND RELEASE OF LIABILITY
6.1 I certify that I am in good physical health and have my physician’s approval to engage in martial arts
training.
6.2 I agree to release Shogen-Ryu Karate-Do International Limited, its officers, directors, instructors,
staff, volunteers, event organizers, facility owners, and associated parties from liability for:
Injuries, death, or property loss resulting from participation, attendance, or travel to and from the dojo
or events. Injuries sustained during competitions, seminars, or other events affiliated with the dojo.
6.3 I acknowledge that the dojo is not responsible for lost or stolen items.
6.4 Misconduct as determined by dojo leadership may result in expulsion without refund.
7. BACKGROUND CHECK POLICY
7.1 For participants 18 years or older working with or near children, a background check may be
required.
7.2 I consent to such checks if applicable.
8. NON-COMPETITION CLAUSE
8.1 I agree not to teach or organize martial arts groups with in Strathroy, Mt Brydges, Adelaide Metcalfe or Kilworth Komoka, competitions, seminars, or other activities that
undermine:
The Canadian Directors of S.K.I.L. Dojo.
Sho Chidu Dojo Headquarters.
Okinawa Shogen Ryu Karate-Do Association.
Marcus Soares Jiu-Jitsu Association.
8.2 Violation of this clause may result in legal action or monetary compensation claims by the affected
parties.
8.3 Any solicitation or recruitment of currently enrolled, inactive, or active students shall be considered a
violation, and appropriate legal action will be pursued.
9. AMENDMENTS AND DISPUTE RESOLUTION
9.1 Any changes to this agreement must be made in writing and signed by both parties.
9.2 Disputes arising from this agreement will be resolved through arbitration in accordance with
Canadian law.
10. ACKNOWLEDGMENT AND BINDING AGREEMENT
By signing this agreement, I affirm that:
I have read, understood, and voluntarily agree to all terms outlined above.
I am legally authorized to enter into this agreement for myself or on behalf of my child(ren).
